7AM-5:30PM with flexibility required Part Time Hours/Wk: Job Open Date: 3/31/23 Application Deadline: Job Duties: The City of Virginia Beach Department of Human Services is seeking to hire a candidate who has supervisor experience and experience working with adults in a ICF/ID Residential LTC for the House Manager position for the Indian River Intermediate Care Facility (ICF).
Duties: As QIDP, the position is responsible for conducting a comprehensive functional assessment, identifying all specific developmental strengths and needs, behavioral management needs, and the need for services regardless of availability.
Develop, in collaboration with the interdisciplinary team and using a person-centered approach, an IPP that identifies discrete, measurable, criterion-based objectives the individual is to achieve, with timetables for expected mastery, and specific specialized and generic strategies and techniques to be used.
The position requires strong advocacy skills.
Provide continuous monitoring of each client’s progress, making modifications to the IPP whenever needed and reviewing the plan at least every 90 days.
Ensure that services provided by each discipline on the individual’s team are integrated within the home as well as the day program, if applicable.
This position acts as the social work designee under the ICF/IID regulations.
Assists in the development and implementation of policies and procedures regarding the operation of an ICF/IID facility.
Works closely with House Managers and nursing staff to ensure that all services in areas, such as active treatment, behavioral and medical, are completed according to procedures and in compliance with State Licensure, Human Rights, and ICF/IID regulations.
Coordinates and chairs the inter-disciplinary team for each assigned resident; gathers appropriate assessment information and develops goals and objectives that can be consistently implemented by both day and residential programs.
Show computer proficiency in Microsoft applications and use of the electronic health record.
Monitor all services delivered to ICF/IID residents to ensure consistency of service, regardless of environment, and implement changes as needed.
Assist supervisors to maintain a safe and healthy physical environment for clients and staff.
Maintain communication with residents, advocates, guardians, and inter-disciplinary team members, and act as social work designee for the facility.

pdf Minimum Requirements: MINIMUMS: DEVELOPMENTAL SERVICES: Requires a four (4) year degree in a human services field (psychology, mental health, special education, social work) and three (3) years experience working with persons with intellectual disability OR experience and/or training equivalent to seven (7) years in fields providing the required knowledge, skills, and abilities for working with persons with intellectual disabilities.
Must have, or obtain, a current and valid drivers license.
SPECIAL REQUIREMENTS: DEVELOPMENTAL SERVICES POSITIONS: Professional licensure as a QDDP (Qualified Developmental Disabilities Professional) may be required for some positions.
