Admissions, Discharge, and Transfers (ADT) Nurse

This position is considered an essential position and therefore must be available to respond to all emergency situations.

Keeping our admissions, discharges, and transfers well organized and complete.

ADT Nurse Minimum Requirements: A current, unrestricted LPN or RN license in the Commonwealth of Virginia.
Proficient with MS Office Suit, TEAMS, EPIC, PCC, and familiarity with web-based collaboration platforms.
A minimum of 1 year of nursing experience is highly desired.
Ability to work independently and collaboratively to manage competing priorities on the unit.
Position Summary: The Admissions, Discharge, and Transfer (ADT) Nurse is responsible for preparing for all new residents admitting to the Seaside unit.
They will ensure all discharge documents are prepared and collaborate with the social worker and therapy departments to keep all critical parties informed of the process.
The ADT nurse will also review and collaborate with the greater team to track transfers on and off the unit and monitor for possible readmission.
In addition to the primary responsibilities this nurse will assist with clinical administrative support related to managing the flow of traffic on the unit by facilitating a smooth flow of communication among the nursing staff, providers, family members, visitors, and vendors.
This position reports to the Unit Manager.
Essential Duties and Responsibilities include the following; other duties may be assigned.
Leads the admission process for all new residents to Seaside between his/her hours of work.
Greets new residents and families with a positive demeanor and coordinates CNA tasks related to admission.
Must be organized and informative as this person is the first impression of the new resident and their families.
Must coordinate with admitting provider to complete initial plan of care.
Reviews acute clinical records for plan of care, identifies complex medical orders, and arranges for needed equipment, i.
e.
air mattress, oxygen concentrator, wound vac, etc.
Enters all admission orders into the electronic medical record, including but not limited to; standing admission orders, new orders for plan of care, all CNA tasks, medications, weight bearing status, diet, labs, follow up appointments, special instructions, etc.
Performs and documents all admission assessments the day of admission, including, but not limited to; a complete, detailed head to toe physical assessment, complete skin examination, resident preferences, fall risk, etc.
and writes a comprehensive admission note.
Performs and documents all wound care upon admission to correlate with the admission orders.
Prepare, fax, and receive prescription information from pharmacy for medications to avoid delays in care upon admission.
Assist with updating 24 Hour Report sheets.
Reviews resident discharge dates and coordinates nursing assessments and tasks with our discharge team to ensure all paperwork is complete on day of discharge.
Oversees resident medical appointment requests by entering orders, completing medical request forms, coordinating with concierge for transportation, etc.
Assist nursing in emergency situations (i.
e.
911 transports, change in medical status) by providing any needed documentation support, communication, and physical assistance (assessments, CPR, suctioning, etc.
) Manage resident lab book by completing lab requisition slips to ensure labs are done, results obtained, and provider is notified of critical results needing immediate attention.
Schedule x-rays, ultrasounds, EKG’s, etc.
with radiology vendor to support the nursing team.
Manage nurse’s station phone calls responding in a timely manner to incoming calls of family members, visitors, vendors (i.
e.
pharmacy, lab, radiology, and hospitals), etc.
Maintains an adequate stock of labs draw supplies, vaccines, and some pharmacy related items.
Assists as requested with any compliance needs on the unit; performs medication and treatment cart audits, reviews medication records for completeness, ensures accuracy in the transcription of physician orders, and adherence to stop-order policies, etc.
Communicates to on call manager any problems / issues brought to attention by staff, residents, family members, etc.
Maintains resident confidentiality and privacy at all times according to policies and procedures; uses discretion when discussing confidential matters.
Maintains positive working relationships with management and coworkers; willingly follows directions and requests as appropriate.
Performs additional tasks or assumes additional responsibilities as assigned by supervisor.
